All insurance co-payments are due at the time of your service.

Benefits will vary depending on the type of policy that you have.  Below are a few questions you may want to ask your insurance:

• If you have a deductible, how much is it?
• If you have a deductible how much has been met for the year?
• What percent of the allowable charge will my insurance cover after I meet my deductible?
• Is there a limit?
• Do you need authorization or a referral?
